FEES & INSURANCE
PALS is an out-of-network provider and does not bill insurance companies directly. However, we can provide a Superbill upon request to help you seek reimbursement. Many insurance plans require preauthorization before in-person testing, so please check with your insurance provider to confirm what is needed for your plan. If your insurance requires preauthorization or additional documentation, please let your PALS provider know exactly what forms or information are needed. Our team can then help you complete and submit the necessary paperwork to your insurance company.
Before in-person testing, your PALS provider will give you a Welcome Letter and a Good Faith Estimate. These documents include essential information for your insurance provider, such as CPT codes, NPI numbers, the EIN, and diagnosis codes. It is essential you contact your insurance company for any questions regarding coverage.
Our comprehensive evaluations are tailored to meet the unique concerns of each individual, leading to varied pricing. The estimated cost for evaluation ranges from $1,500 to $2,400, with most families paying between $1,800 and $1,950. Before testing begins, you will receive a Good Faith Estimate outlining the specific cost. Reevaluations are available for clients who completed their initial evaluation with PALS and are seeking updated testing. Payment Schedule
A $150 non-refundable deposit is required to reserve your evaluation and will be applied to your total bill. Half of the remaining balance is due before in-person testing, and the final balance is due one business day before your Feedback Appointment.
